The lawsuit was filed by Muhammad Syakir, represented by his father and litigation representative, Syed Ali Syed Hussein, due to the school's failure to provide reasonable supervision during a physical and health education class. The case named multiple defendants, including the school activity teacher, Nur Badruzzanan Mohamad, the school principal, Maziah Mohammad, the Education Director-General, and the Malaysian Government. The incident occurred when Nur Badruzzanan retrieved sporting equipment and conducted a frisbee activity, after which Muhammad Syakir Irfan obtained a school-issued badminton racket to play an informal game with a classmate.
